P0121 indicates that the throttle position sensor (TPS) or accelerator pedal position sensor circuit is reporting a voltage, angle, or performance value outside the expected range, or that the throttle plate position does not match what the engine computer expects. It is most commonly a sensor, wiring, or throttle-body issue rather than a computer failure.

Symptoms

What causes it — most likely first

1. Faulty throttle position sensor

The TPS internal resistor track or contacts can wear, causing voltage dropouts, dead spots, or an out-of-range signal as the throttle opens and closes.

2. Dirty or binding throttle body

Carbon buildup or a sticking throttle plate can make the actual blade position differ from the commanded or sensed position, triggering a range/performance fault.

3. Wiring or connector faults in the TPS circuit

Chafed wires, corroded terminals, loose pins, or poor grounds can disturb the 5-volt reference, signal return, or ground circuit and produce erratic readings.

4. Throttle linkage or cable misadjustment

On cable-actuated throttles, binding, worn bushings, or incorrect adjustment can cause the TPS reading to disagree with the actual throttle plate angle.

5. Accelerator pedal position sensor fault

On drive-by-wire systems, an inconsistent APP sensor signal can contribute to throttle correlation faults, though it more commonly sets related APP-specific codes.

6. PCM/ECM hardware or software fault

A failed engine computer is a rare cause of P0121 and should only be considered after the sensor, wiring, and mechanical causes have been thoroughly ruled out.

How it's diagnosed

  1. Retrieve all stored codes and freeze-frame data; note any related TPS, APP, or throttle actuator codes.
  2. Visually inspect the throttle body, blade, linkage, and TPS connector for carbon buildup, damage, corrosion, or loose terminals.
  3. With a scan tool, monitor TPS voltage or percentage as the throttle is slowly opened and closed; look for dropouts, spikes, or dead spots.
  4. Back-probe the TPS connector and check reference voltage, ground, and signal output against manufacturer specifications with a digital multimeter or oscilloscope.
  5. Verify the throttle plate moves smoothly through its full range and that the sensor mounting is secure and properly aligned.
  6. If all mechanical and electrical tests pass, consider a PCM software update or, only as a last resort, PCM replacement.
