P0192 indicates that the powertrain control module is detecting a voltage signal from the Bank 1 fuel rail pressure sensor that is below the expected minimum threshold. This is an electrical circuit fault, meaning the sensor's signal wire, reference voltage, ground, or the sensor itself is not producing the expected voltage range.

1. Failed fuel rail pressure sensor

The sensor itself is the most common failure point. Internal degradation can cause the signal output to drop below the valid range or stick at a low voltage.

2. Wiring or connector fault

Corrosion, chafed insulation, bent terminals, or a poor connection at the sensor pigtail can cause an open circuit or high resistance on the signal, reference, or ground wire.

3. Signal circuit shorted to ground

If the signal wire between the sensor and the PCM is shorted to ground, the PCM will read near-zero volts, triggering the low-voltage code.

4. 5V reference voltage supply problem

The sensor requires a stable 5-volt reference from the PCM. A shared reference circuit fault (affecting multiple sensors) or a wiring issue on the reference line can cause the sensor output to read low.

5. PCM/ECM internal fault

Extremely rare. The PCM's internal voltage reference regulator or input circuit would have to fail. This should only be considered after all wiring, connector, and sensor tests have conclusively passed.

How it's diagnosed

  1. Read all stored codes and freeze frame data to confirm P0192 is current and check for related sensor codes.
  2. Visually inspect the fuel rail pressure sensor connector and harness for corrosion, bent pins, chafing, or loose terminals.
  3. With the key on and engine off, backprobe the sensor connector and verify 5V reference voltage and a good ground at the appropriate terminals.
  4. Check the signal wire for continuity to the PCM and for a short to ground using a multimeter; repair any wiring faults found.
  5. Compare the sensor's reported fuel rail pressure on a scan tool against a mechanical fuel pressure gauge reading to determine if the sensor is reading inaccurately.
  6. If reference voltage, ground, and wiring all test good but the signal remains low, replace the sensor. Only evaluate the PCM if the new sensor and verified wiring still produce the same low-voltage code.
