5. Faulty EGR feedback sensor (DPFE, EGR temperature sensor, or MAP sensor)

A sensor that incorrectly reports EGR flow can cause the ECM to set a false P0401 code even if the mechanical system is working properly.

6. Wiring or connector issues

Damaged wiring or corroded connectors in the EGR control or sensor circuits can prevent proper operation or feedback.
